Pro Se Litigant - Filing a New Case

Before you proceed with filing a new case, refer to the checklist below to ensure a successful filing.

  • Civil Cover Sheet
  • Complaint
  • Motion for Leave to Proceed In Forma Pauperis (if applicable)

Once the new case filing is accepted, you will receive a notice electronic filing directing you to pay the filing fee. You may pay your fee by mailing a cashier's check or money order to: U.S. District Court, P.O. Box 607, Muskogee, OK 74402. You may also pay your fee with cash or card at our clerk's office counter located at: 101 N. 5th Street, Room 208, Muskogee, OK 74401. No personal checks will be accepted.
